I hereby declare that all personal information and other particulars furnished by me in this application form are true and correct to the best of my knowledge. I have been informed of the course fee and the mode of payment. In the event that any information furnished by me to RAK TRAINING SHIP SINDBAD FZ LLC - CENTER FOR MARITIME STUDIES AND SHIP MANAGEMENT (herein after called RAKTSS) is found to be false or incorrect, RAKTSS reserves the right to suspend or expel me from the course and forfeit my fee.
In the event that I willfully and /or prematurely withdraw my name form the course , I understand that I shall have no right to a fee refund.
I affirm that I have read and understood the terms of acceptance and code of conduct. I agree to abide by the rules and regulations, maintain discipline and conduct myself in a manner befitting a cadet of RAJTSS, in the event of any breach of the acceptance terms and conditions for this course, RAKTSS reserves the right to discharge me from the course and forfeit my fee.
I understand and agree that all disciplinary and other matters concerning the above shall be referred to the disciplinary committee and I shall agree and abide by the decisions of this committee.
I also understand that during my training with RAKTSS. I shall abide by the laws, rules and regulations of the government of UAE.
